Safety Practices include but are not limited to hand-hygiene; and
for direct and indirect patient care Associates: accurate patient
identification (e.g. prior to transport, procedures, medication
administration, registration etc), hand-off communication,
read-back, appropriate specimen labeling in presence of patient,
MRI safety etc. Takes responsibility for Associate Safety: Follows
workplace safety and health guidelines, reports hazardous
conditions and concerns, wears protective equipment as prescribed.
Care provided is appropriate to the age of the patients served.
Demonstrates the ability to obtain information and interpret
information in terms of the patient s needs. Demonstrates
appropriate knowledge of growth and development. Demonstrates an
understanding of the range of treatment needed by these patients.
Makes initial assessment, and determines appropriate nursing needs
of assigned patients, as evidenced by appropriate documentation.
Performs initial and ongoing assessment on all assigned patients,
including a review of systems, as evidenced by appropriate
documentation. Functions in first-line management/clinical
leadership role in coordinating patient care activities on the
nursing unit. Participation in huddles Provides for direct and
indirect nursing care to meet the physiological, psychosocial, and
spiritual needs of the patient and significant others. Is
knowledgeable of current nursing practices and ensures compliance
to the hospital's policies and procedures. Serves as a coordinator
of the multi-disciplinary approach to patient s care to include
care planning, patient teaching, and discharge planning. Assists
with patient/family education as needed. Variance reports are
completed within the defined time frame. Ensures appropriate
staffing and coverage for unanticipated needs, within the
guidelines of the department. Assists in long and short-term staff
scheduling. Participates in the interviewing process of prospective
staff and provides input to the Director. Ensures that staff
utilize the nursing process in meeting patient care standards.
Utilizes effective delegation skills in assigning duties to other
staff and Senior Nurses. Demonstrates proficiency in the provision
of nursing care. Insures appropriate patient preparation as well as
completion of chart, prior to surgery, special procedure, transfer
or discharge. Administers medications and treatments as ordered by
the physician and in accordance with Policy and Procedure.
Establishes nursing care priorities. Competently cares for patient
with highest acuity on unit (unit specific). Provides, or
supervises, patient s care and treatment. Performs other
job-related duties within job scope as requested by the Nurse
Manager. Assists fellow staff members in completing assignments in
time of crisis. Assumes primary nurse, team leader, or patient care
responsibilities as necessary. Promotes teamwork and collegial
relationships with, and among staff. Facilitates daily huddles and
discussions around huddle boards Facilitates debriefs after
critical events such as patient falls to improve learning Receives
pertinent information from acute care management and follows up on
any needs during off hours. Communicates clearly and effectively
with physicians and other staff health care providers. Demonstrates
effective communication skills. Intervenes appropriately and keeps
the Nurse Manager informed of departmental issues. Follows
appropriate lines of communication and appropriately documents
actions taken, i.e: patient chart, anecdotal note, variance report.
Responds to physician and staff issues in a timely manner.
Schedules, conducts, and/or documents monthly staff meetings, as
directed by the Unit Director. Demonstrates the essence of team by
acting in a positive, productive manner with co-workers and other
departments personnel. Serves as role model to other staff members.
Provides input Associate evaluations as requested by the Nurse
Manager. Provides feedback to Associate related to performance and
documents same. Conducts initial investigations of untoward events
and provides initial coaching, with feedback shared with Nurse
Manager. Maintains, and works with, staff throughout the year in
achieving performance improvement goals. Inspect unit for
compliance with unit/departmental policies, procedures,
regulations, and standards. Takes measures to correct and report
when violations occur. Reads, and complies with, established
Hospital Policies and Procedures and department memos. Maintains,
with compliance to, all requirements elated to JCAHO, AHCA, HIPAA
and other regulatory agencies. Serves as a coach/mentor in
recommending personnel actions such as promotions, clinical ladder
advancement and transfers. Ensures Associate mandatory requirements
are completed on time. Monitor workflow and productivity during
shift on unit. Assure cost-effective utilization of supplies and
equipment. Assure adequate quantity of supplies and proper
functioning equipment. Coordinate and adjust work schedules to meet
