setting within the last ten years. - Experience conducting
comprehensive assessments of patients. - Ability to work with

ability to work independently. Description: Required Licensure:
Current RN Licensure in New York State. Required Experience: Two
years of experience in a home setting within the last ten years.
Nurses must have recent experience conducting comprehensive
assessments of patients. All nurse assessors must meet the
following requirements. Experience may include working in the field
for a Certified Home Health Agency or similar services. Conducting
assessments similar to UAS such as OASIS is necessary. Providing
primary care to homebound persons and care management for those
receiving home and community-based services is essential. Home care
experience as a paraprofessional or Licensed Practical Nurse caring
for a minimum of two patients per week is acceptable. Preferred:
Additional experience represents the gold standard for this
position. Experience conducting assessments in UAS-NY, especially
in the last 365 days, is advantageous. Experience caring for a
geriatric population and managing chronic diseases in the home
setting is beneficial. Ability to review health status questions
and make evaluations based on UAS criteria for enrollment. Ability
to perform initial in-home pre-evaluation of patients, including
evaluation of health status, strengths, care needs, and
preferences. Review consumer medical documentation and/or health
referral forms as relevant to the case. Enter evaluation data into
electronic evaluation forms and transmit as required. Document any
concerns, conflicting information, or other issues that arise
during the evaluation process. Emphasize continuity of care,
reducing fragmentation, duplication, and gaps in treatment plans.
Discuss with consumers healthcare options, supports needed, service
vendor options, and waiver options. Initiate communication for
enrollment into a managed care health plan. Enhance communication
and collaborative relationships with interdisciplinary care team
members to improve care coordination. Collect quality review data
and required documentation to support outcome measurements and
record case notes. Identify opportunities for health promotion and
illness prevention. Maintain a comprehensive working knowledge of
community resources and payor requirements. Meet all standards
established for this position as outlined in the corresponding
annual performance criteria. Perform other duties as may be
assigned by the Regional Nurse Manager or other management. 5
