Job Description:
Sunrise Senior Living is seeking a compassionate and experienced
LPN Med Care Manager to join our dedicated healthcare team. This
role is pivotal for providing high-quality nursing care and daily
medical management to our senior residents, in alignment with our
commitment to personalized senior care services. The LPN Med Care
Manager plays a critical role in our mission to offer a nurturing
environment and individualized care to enhance the well-being of
our residents. Administer daily medication and treatments to
residents, and monitor outcomes. Coordinate with healthcare
providers to ensure comprehensive resident care. Assist in the
development and implementation of care plans tailored to individual
resident needs. Educate residents and their families on health
maintenance and disease prevention. Maintain accurate and
confidential resident medical records. Participate in the training
of new staff and ongoing in-services for existing staff. Ensure
compliance with all federal, state, and local regulatory
requirements. Proven ability to manage and administer medications.
Strong communication and interpersonal skills. Organizational
skills with an aptitude for multi-tasking and prioritizing work.
Experience in patient health assessment and care planning. Medical
Insurance Dental Insurance Vision Insurance Life Insurance 401k
Valid Licensed Practical Nurse (LPN) license in the state of
practice. CPR and First Aid certification preferred.
