Job Overview The Patient Navigator will assess patients with substance use disorder for psychosocial needs that may contribue to their substance abuse. The navigator should be someone knowledgeable about substance use disorder and of the community cluture and languages spoken. The Navigator will provide accurate health-based information geared towards addressing structural racism and reducing identified health disparities. Additionally, they will ensure information regarding appropriate community services is made avaialble to individual interested in initiating or sustaining recovery. Client outreach/follow-up will occurin in the Addiction Medicine office. The Navigator will work alongside an addiction medicine physician to engage clients with substance use disorder for medical care and wrap around services. Spanish speaking preferred. Qualifications Associates degree with 4 years of clinical/office or lived experience. Certified Medical Assistant with 4 years of clinical/office or lived experience. Bachelor's degree with 2 years of clinical/office or lived experience.
