Summary: To provide intensive clinical case management services for
families affected by parental and/or child mental health and/or
substance abuse, trauma, or intimate partner violence issues that
place the family at risk of foster care placement. Duties and
Responsibilities: Assess and refer children/individual/families to
supportive and concrete services, housing, financial assistance,
education, substance use, mental health, medical, immigration,
legal, etc. Provide case planning, assessment, crisis intervention,
establish goals and objectives and counseling services for families
who have a child at risk of Foster Care placement. Conduct
assessments of families within 30 days of intake and reviewed
review and update in 6months after intake. Participate and attend
all ACS, OCFS, Agency and Model Developer trainings as requested or
required. Provide crisis intervention and counseling services to
children, teens, individuals and families. Collaborate with program
case aide in supporting families in obtaining resources, support
services, and advocacy assistance. Collaborate with ACS to monitor
compliance with mandated services, and family attendance of court
hearings. Collaborate with ACS legal team to assure all service
needs are being addressed by families. Document and maintains all
mandated tracking logs/excel sheets, case record electronic
systems, and document in a timely manner. Complete all program
required paperwork, i.e., Progress Notes, Program Family
Assessments, Family Assessment Service Plans (FASPs), and other
reporting requirements. Request, participate, facilitate and or
attended, conferences, meetings internal or external to the agency.
Assist individuals and families in developing services plans
addressing safety, stabilization, substance use, mental health,
domestic violence, from a harm reduction approach. Participate in
weekly Supervision, with a planned agenda on family case issues to
discuss· Implement Quality Assurance data to drive case practice
and improve performance and achieve outcomes. Monitor and obtain
information on the child/ren school placement and educational
performance. Monitor and obtain information on the family’s medical
needs, including compliance with periodic physicals and
immunizations. Assist in the obtaining, gathering and data entry
for CACFP/DOH/DOE and any other administrative governing body.
Perform other duties/responsibilities as assigned within scope of
position Requirements: Skills, Knowledge, Abilities: Strong
communication skills, with the ability to convey and receive
accurate information in a professional and empathetic manner.
Ability to collaborate effectively with supervisors, colleagues,
and community partners. Excellent organizational and time
management skills to handle caseloads efficiently while managing
emergent situations. Skilled in conducting interviews with families
to gather and provide necessary case management information. Strong
critical thinking and analytical skills to develop and implement
achievable service plans with families. Ability to perform
essential job functions with or without reasonable accommodation.
Effective collaboration and facilitation skills to engage
individuals, families, and service providers. Demonstrated cultural
competence and sensitivity in working with diverse families from
various ethnic, socioeconomic, and cultural backgrounds.
Qualifications/Requirements: Bachelor’s required Language incentive
.28/hr - Spanish speaking Experience with Microsoft Word, Excel,
PowerPoint, CNNX, PROMIS?? At least two years child welfare and
counseling experience preferred.? Due to the nature of the work
environment work hours will be based on needs of program and may
